Abstract
Purpose: The purpose of this project was to determine the effectiveness of the Seek, Help, Assess, Reach, and Evaluate (SHARE) approach in the management of cardiovascular disease (CVD) risk factors in the Hispanic population at a primary care clinic while improving health literacy. The aim was that by February 2025, Hispanic patients at the clinic would achieve a 10% reduction in CVD risk factors.
Background: The project was implemented at a small private primary care clinic in Texas. CVD risk factors prevalent include hypertension, mixed hyperlipidemia, and diabetes mellitus.
Methodology: Before implementation provider and staff were educated on the SHARE approach. The PDSA framework was used to guide the SHARE approach, and pre and post assessment health survey questions. The implementation included 26 Hispanic patients aged 25 and older with at least one CVD risk factor were gathered. Data collected included demographics, medical history, laboratory results, and patient pre and post survey answers.
Results: With a decrease in overall CVD risk factors showed improvement with percentage decreases in blood pressure(3.47% SBP and 3.51% DBP), HbA1c (10.04%) cholesterol (7.83%) and triglycerides (33.99%) post-intervention. Findings suggested the value of incorporating patient preferences and values into care delivery.
Implications: The SHARE approach showed promise in improving patient engagement and knowledge. Challenges such as patient appointment adherence was noted, suggesting the need for ongoing support in appointment reminders. Continued emphasis on patient education and self-management strategies is critical to sustaining long-term improvements in CVD risk factors.
